In 2021, nearly 1.9 million new cases of cancer were projected to be diagnosed in the United States (1). And many Americans living today are survivors of invasive cancer. Statistics published in 2021 show that about 16.9 million Americans living as of January 1, 2019 had a history of invasive cancer. The majority of these Americans, fortunately, had their diagnoses long in the past and have no current signs of cancer (2).
All in all, the U.S. has seen a 27 percent reduction in cancer death rates between 2001 and 2020 (3). These statistics are obviously very encouraging for public health and for people confronting a cancer diagnosis.
Researchers attribute the encouraging long-term trends in the cancer death rate in the United States in part to reduced smoking rates in the country. Throughout most of the 20th century, the age-adjusted rate of death from cancer in the United States increased, reaching an apex in 1991 when it measured 215 deaths per 100,000 people. As smoking rates dropped, this trend reversed, with the U.S. age-adjusted cancer death rate decreasing to 149 per 100,000 people by 2018 (4).
Along with reduced smoking rates in the population, researchers have also credited several other factors for the declining cancer death rate. These include changes in the risk profile of the U.S. population, the rising prevalence of screening tests, advances in earlier diagnosis and improved treatments for certain types of cancer (5).
Most importantly, in human terms, the positive statistical trends mean about 3.2 million fewer people died of cancer in the U.S. between 1991 and 2018 than would have if earlier cancer death rate trends had continued (6). Clearly, long-term trends in cancer and holistic cancer care provide an optimistic outlook as the big picture for cancer survival improves over time.
Every Patient is Unique
Major statistical trends like those discussed above combine all cancer patients into one group. However, it’s essential to note that every case of cancer is unique and each treatment program must be tailored to the individual patient. The basic stage is always described based on the characteristics of the cancer when it is first diagnosed. Even if the cancer changes or spreads later, the basic stage doesn’t change. However, additional descriptive information may be added to the initial staging category at diagnosis to help provide a shorthand way of communicating the cancer’s evolution over time.
The TNM staging system is used for staging many different kinds of cancer. Here’s how to understand this common staging system for cancer:
- T stands for tumor and is used to describe the main, or primary, tumor’s size.
- N stands for nearby lymph nodes and describes how many lymph nodes adjacent to the primary tumor contain cancer.
- M stands for metastasis and describes whether or not the cancer has spread from its first location, the primary tumor, to additional areas in the body.
For each letter in the TNM staging system, numbers and letters provided after the T, N and M will offer more details about each element of the cancer (8).
Certain types of cancer, such as blood cancers and tumors of the spinal cord and brain, have their own dedicated staging systems better suited to describe how those particular types of cancers can vary from case to case. However, like the TNM system, any cancer staging system offers healthcare providers a way to encapsulate briefly the main characteristics of a patient’s cancer that are relevant in determining the best course of treatment.
