Hereditary vs. Sporadic Bladder Cancer
Doctors generally group cancers into two categories: hereditary and sporadic.
- Hereditary bladder cancer is linked to an inherited gene change that is passed from parent to child. These genetic variants are present from birth and can increase the risk of developing bladder cancer and other cancers.
- Sporadic bladder cancer, on the other hand, develops because of genetic changes that occur during a person’s lifetime. These changes can happen as cells in the bladder divide and age, accumulating DNA damage, often after years of exposure to factors such as tobacco smoke or certain chemicals, until a tumor eventually forms. How fast bladder cancer spreads depends on the type, grade, and stage of the cancer, rather than whether it is hereditary or sporadic. Large-scale genomic research, including work from the Cancer Genome Atlas, has identified specific gene mutations, such as changes in the FGFR gene, that are commonly associated with bladder cancer.
While inherited gene changes can contribute to a genetic predisposition in some families, they account for only a small percentage of bladder cancer cases. Cancer research groups such as the National Cancer Institute note that most cancers, including bladder cancer, develop from an accumulation of genetic changes over a person’s lifetime rather than a single inherited cause.
Does Bladder Cancer Run in Families?
Although bladder cancer does not usually run in families, having a family history of bladder cancer can modestly increase your risk. That doesn’t necessarily mean bladder cancer is hereditary in your case.
Having a First-Degree Relative With Bladder Cancer
A first-degree relative is a parent, sibling, or child.
Research suggests that people with a first-degree relative who has had bladder cancer are more likely to develop bladder cancer themselves.2 Even so, most people with an affected first-degree relative will never develop bladder cancer.
Instead of causing alarm, a family history should be viewed as one piece of your overall risk profile. Your healthcare provider may consider it alongside your age, smoking history, occupational exposures, and other factors when discussing further evaluation.
Shared Lifestyle and Environmental Factors
In many families, people share more than genes. They often share lifestyles and environmental factors, which can also contribute to similar cancer patterns across relatives.
For example, cigarette smoking remains the leading risk factor, and working in industries can involve long-term exposure to harmful chemicals that may lead to bladder cancer.3 When smoking is a household habit, or several relatives work in the same family business or industry, it can be hard to distinguish between inherited and shared environmental risk factors.
This is one reason doctors consider family history alongside other factors rather than treating it as proof of an inherited cancer syndrome.
Genetic Conditions Associated With Bladder Cancer Risk
A few inherited cancer syndromes can create a higher risk of bladder cancer and other urinary tract cancers because they are passed down through generations. They’re uncommon and are responsible for only a small share of cases of bladder cancer overall, but may be meaningful when they run in a family.
Lynch Syndrome and Urinary Tract Cancer
Lynch syndrome, also known as hereditary nonpolyposis colon cancer, is an inherited condition best known for increasing the risk of colorectal and endometrial cancers. Lynch syndrome results from inherited mutations in genes such as MSH2, which normally help repair errors in DNA. It is also associated with a higher risk of certain urinary tract cancers, particularly cancers of the ureter and renal pelvis, the areas where urine is filtered by the kidneys and carried to the bladder, and may increase the risk of bladder cancer in some people.4
Having bladder cancer alone does not mean you have Lynch syndrome. Genetic testing is typically considered only when a person’s personal or family history suggests an inherited cancer syndrome.
Other Rare Inherited Associations
A small number of other inherited genetic conditions may also create a predisposition to bladder cancer or other urinary tract cancers. These are rare and are usually suspected only when there is:
- A strong family history
- Multiple related cancers in the same family
- Cancers diagnosed at unusually young ages
Bladder Cancer Risk Factors Beyond Genetics
Genes are only part of the picture. Most bladder cancers are linked to non-genetic factors, which is why someone with no family history can still get bladder cancer. Even non-smokers can develop bladder cancer, though smokers face a substantially higher risk. Regardless of your family history, being aware of common bladder cancer symptoms can help you recognize when it may be time to speak with a healthcare provider.
The biggest risk factors for bladder cancer include smoking, older age, chronic bladder irritation from conditions such as bladder stones or urinary tract infections, and exposure to certain industrial chemicals. According to the American Cancer Society, about 9 out of 10 people diagnosed with bladder cancer are older than the age of 55. When Might Genetic Counseling Be Considered?
Genetic counseling isn’t recommended for everyone with bladder cancer. Instead, it’s usually considered when your personal or family history suggests an inherited cancer syndrome.5
- Multiple related cancers in the family – A pattern of bladder cancer, upper urinary tract cancer, colorectal cancer, endometrial cancer, or other Lynch syndrome-related cancers in several close relatives may suggest an inherited cancer syndrome. However, this doesn’t automatically mean a bladder cancer genetic condition is present.
- Cancer is diagnosed at an unusually young age – Bladder cancer is more common in older adults, so a diagnosis at a younger-than-expected age may be another reason to take a closer look at your family history.
What Genetic Testing Can and Cannot Tell You
Genetic testing looks for inherited gene changes that may increase your genetic risk for certain cancers.
- A positive result means an inherited genetic variant was found. It does not mean you will definitely develop cancer.
- A negative result means no known inherited variant was identified, but it doesn’t eliminate the possibility of developing bladder cancer.
Sometimes, testing finds a variant of uncertain significance (VUS). This means a genetic change was detected, but researchers don’t yet know whether it affects cancer risk.6
No genetic test can predict with certainty whether someone will or won’t develop bladder cancer, since it only provides information about inherited risk.

- The type of cancer each relative had
- Their age at diagnosis, if known
